Social connector

Council’s Community Care Services provides support options for people (and their carers) with difficulties performing daily living tasks due to disability or frailty. This assists people to stay active, remain independent community members and avoid the need for early residential care. 

Funding for an information/connector service delivered by Council to assist older people and people with disabilities and their carers to navigate and access appropriate MyAgedCare, NDIS and HACC-PYP services, including those delivered by Council and by other local providers.
